This Chimney Fire was probably caused by either burning un-seasoned wood or not having the chimney swept often enough (or possibly both).

The second picture shows a chimney which has been involved in a chimney fire. You can see that the tar inside the chimney has "bubbled" resulting in a much reduced area for the smoke to pass up through the flue.
